Problem Item Report Form

While using the Measures of Academic Progress® (MAP) system, you may occasionally see test items that have display or content issues. We request your assistance in reporting these items to NWEA. By doing so, we can better ensure the quality of our MAP tests. Please report any questionable items by providing as much information as possible in this Problem Item Report Form.

After filling out the form, please save a copy and send it to ItemReport@nwea.org.

All Problem Item Reports are reviewed by an NWEA specialist, and when applicable, corrected items are made available in subsequent test package releases. If we have questions about your submission, an NWEA representative may contact you for further details.
